Who can Apply?
- Holders of a long-term visa (except short-term, seasonal, or special work visas) and has been in the Czech Republic for not less than 6 months
- Holders of a Long-term Residence Permit

- In person at the Ministry of the Interior
- Minors under 15 must apply through a legal representative (ex. a parent)

- CZK 2,500 (adults)
- CZK 1,000 (minors under 15)

Who can Apply?
- spouse of a residence permit holder/employee card holder with at least 6 months living in the Czech teritories
- A minor or dependent adult of a residence permit holder or their spouse
- An adopted child by a residence permit holder or their spouse
- A single family member dependent to the residence permit holder due to health reasons (ex. a parent)
Application Process
Applications must be submitted at the Czech Embassy or Consulate.
The Ministry of the Interior evaluates the application. If approved, a long-term visa is issued for collecting the residence permit upon arrival in the Czech Republic.

- Two passport photos
- Biometric data (fingerprints)
- Fee
- Proof of family relationship (birth certificate, marriage certificate, etc.)
- Proof of financial income
- Proof of accommodation (e.g., rental agreement)
- Criminal record:
- Filipino applicants: NBI clearance (authenticated, apostilled, and translated into Czech).
- Parental consent (if applicable, for minors)
- Health insurance (if required for visa issuance)
- Proof of disease prevention measures (if requested)
How to Apply
Email manila.visa@mzv.gov.cz with your name and proof of family relationship (e.g., birth certificate) to request an appointment. Applications are processed on a first-come, first-served basis.
